If you would like your child(ren) to be added to the birthday club for future promotions/offers given for an upcomnig birthday, please enter the names and birthdates of each of your children as well as an email address as to where you would like the special sent. The birthday club is available to all children ages 3 months to 18 years of age. I offer specials for 3 months, 6 months, 9 months, 1 year, 18 months, 2 years and then every year after that until your child reaches their 18th birthday.
